The tumor is large and will be removed next week. He has some “spots” in lungs, but it is unknown if they are actually metastatic lesions. What should be the course of action after surgery?

It is very encouraging that his team has opted to proceed with the surgery (nephrectomy) which suggests to me that they do not suspect the lung “spots” to be metastatic from his renal cancer or else they would have advised treatment with medications (targeted therapies). A better answer will be available after surgery when his team will know the exact type and extent of his tumor. Treatment after surgery is multifactorial including recovery, diet changes, and monitoring. Regarding whether he will need medications after surgery for a localized tumor, the standard of care remains observation for now (although there are some clinical trials looking into whether treating with targeted therapies after surgery is of benefit, but the data thus far has been negative).
